Story spoilers
In the manga, Ruka returns after an extended absence caused by illness and pushes Juri toward another duel with Utena. The strain of his own duel triggers a relapse and he later dies after revealing that he wanted Juri to obtain Dios's power. The anime instead has him manipulate Shiori, court Juri, and later appear as Juri's Rose Bride; his reported death there is left more ambiguous.
