Story spoilers
Nanako is actually a clone, the latest in a line created from an earlier woman of the same name who died from a mysterious illness at twenty. Every clone has suffered the same fate, and Ogami's harsh training is tied to his effort to find a cure and prevent the current Nanako from dying the same way.
