Story spoilers
Mihoko normally keeps her right eye closed to conceal her heterochromia. Opening both eyes during difficult mahjong games sharpens her perception of opponents' discards and plans. She also recognizes Hisa Takei from a tournament three years earlier and continues to admire her after Hisa once called her hidden eye "pretty."
