Story spoilers
In the manga, Kazuna initially loves Yaegashi but distances himself after dreaming that he kills her for blood, then gradually develops romantic feelings for Chizuna. As Chizuna nears death, he says he will take the suicide medicine she gave him because he cannot imagine living without her. In the anime he dies; in the manga he is shown in a hospital with no memory of the previous year.
