After their mother's death, she agrees to train the brothers through a demanding combination of alchemy, philosophy, martial arts, and wilderness survival. Story spoilers
Years earlier, Izumi and her husband Sig lost their stillborn child. Izumi attempted human transmutation to revive the child, and the failed act cost her internal organs while also allowing her to perform alchemy without a transmutation circle after seeing the "truth." Later, Hohenheim rearranges her insides to improve her blood flow, and she agrees to help oppose the State Military.
